Left Cranial Lung Lobe Torsion in a Dog Diagnosed by Imaging: A Case Report

Abstract

Lung lobe torsion in dogs, particularly in brachycephalic breeds, is a complex and potentially lifethreatening pathology in canines, characterized by an abnormal rotation of a pulmonary lobe around its hilum. This condition compromises the vascularization and lymphatic drainage of the affected lobe, inducing ischemia, edema and, eventually, tissue necrosis. The clinical signs are often nonspecific and can be confused with other respiratory diseases, making early diagnosis difficult. In this study we present the case of a Pug dog that was evaluated for an acute onset of respiratory disease, and in which a computed tomography (CT) was diagnosed with torsion of the left cranial pulmonary lobe. CT was an essential diagnostic tool to visualize the anatomical alterations characteristic of this pathology and also to rule out other differential diagnoses. Given the severity of the clinical signs, a surgical approach was chosen, performing a pulmonary lobectomy. Since the patient presented a favorable postoperative evolution, the importance of an early diagnosis  and  a  timely  surgical  management to improve the prognosis is emphasized. It should  also  be considered that pulmonary torsion represents a diagnostic and therapeutic challenge, so CT should be considered as the test of choice to confirm the diagnosis and evaluate the extent of the lesion, likewise surgical treatment, although complex, offers excellent results in these patients.
